Short-Term Rehabilitation

On your admission day, we take the time to carefully evaluate your condition and medical history, then customize a rehabilitation plan and provide you with the care options that best fit your lifestyle and goals, ensuring the highest level of function and safety for returning home.

This recovery plan includes in-depth evaluations by all three disciplines below to determine functional and cognitive areas for improvement:

Physical Therapy

Our team is trained to provide pain management as well as neuromuscular re-education and muscle strengthening, with the goal to improve overall strength and balance for safe ambulation at home and in the community.

Speech Therapy

We help to increase cognitive ability such as memory, sequencing and problem solving. Along with increasing safe swallowing to decrease aspiration risk, speech therapy helps with fall prevention by improving cognitive awareness of fall risk factors.

Occupational Therapy

Through pain management, neuromuscular re-education and muscle strengthening, we help improve quality of life by increasing independence with daily functions. We also specialize in therapy after orthopedic procedures to improve range of motion in the affected area to regain mobility.

We collaborate with residents and their families to determine the needs for residents to return home safely and together, we work toward our established goals within each discipline.

In addition to providing these services for our patients, we provide education to families to improve caregiver abilities and safety for when the patient returns home. As the resident demonstrates improvements and becomes more independent, home visits are scheduled to help ensure a smooth transition.
